Request Assistance

İhtiyaç sahibi ailelerin başvurularını aşağıdaki formdan alıyoruz. Başvurunuz Aile Tespit Birimimize iletilir; birim sizinle telefonla iletişime geçer ve gerekli görülürse ev ziyareti yaparak durumunuzu değerlendirir.

Başvurunuzda verdiğiniz bilgiler yalnızca yardım değerlendirmesi için kullanılır ve üçüncü kişilerle paylaşılmaz. Formu doldurmak yardım taahhüdü anlamına gelmez; değerlendirme sonucu her hâlde size bildirilir.

Acil durumlarda formu beklemeden derneğimizi telefonla arayabilirsiniz.
